When Senator Boxer asked General Walsh to please, call me Senator it was like a flashback to In Heat of the Night wherein Sidney Pointer famously said, "They call me Mr. Tibbs" as the law enforcement of that small Mississippi town was investigating a murder.

This is awesome. Can you imagine the general's reaction if a PFC didn't use his title? Sheesh! You'd think the guy would have a better appreciation for formality and gestures of respect! Oh, wait...respect. Nevermind, it makes sense now.

Appropriately respectful addresses do not require title, just "sir" or "ma'am," which is why the general didn't do anything wrong and wouldn't have a reaction if he were addressed by a PFC as "sir."
The only problem here is the ignorance of military courtesy by Sen Boxer and anyone else who thinks the general did something wrong.
